Medical Forms Needed

Please don't forget to print out the attached medical form, complete it and bring it to practice Thursday, September 13.
Megan will be collecting them at the end of practice to give to the Women's trainers.
Remember, you need these filled out no later than Saturday, September 22 or Tanya won't let you play!
If you don't have a printer, Rae will be bringing out some extra forms for you to fill out.
